PASSPORT

PASSPORT is a category of long-term care services provided to individuals in their homes. Services may include general nursing care, assistance with personal care and activities of daily living, homemaker services such as housekeeping and meal preparation and assistance to “at-risk” individuals.

What is PASSPORT?
PASSPORT is an Ohio Medicaid program providing long-term in-home care services for eligible older Ohioans. The Central Ohio Area Agency on Aging evaluates individuals to determine eligibility, and then a PASSPORT case manager works with them to develop a program for their in-home service. Loving Care is committed to provide quality, personal care to meet specialized needs of clients in this program.

Who is eligible for PASSPORT?
Participant eligibility:

  • be at least 60 years of age
  • be financially eligible for Medicaid institutional care
  • require a nursing home level of care
  • be able to remain in a safe home setting with the consent of their physician

What are the benefits of PASSPORT care?
